**Jackie Belter Fire-Bird-Yoga** **Participant Waiver & Release of Liability** Participant Information Assumption of Risk I understand that participating in classes and services offered by **Jackie Belter of Fire Bird Yoga**, including yoga, power yoga, yoga sculpt, barre, chair yoga, mobility, stretching, breathwork, meditation, workshops, corporate wellness programs, and private sessions, involves physical activity and carries inherent risks of injury. I voluntarily choose to participate and accept all risks associated with these activities. Health Declaration I confirm that I am physically able to participate and will inform the instructor of any injury, medical condition, pregnancy, recent surgery, or other health concern that may affect my participation. I understand it is my responsibility to work within my own limits and stop if I experience pain, dizziness, or discomfort. Release of Liability I release and hold harmless **Jackie Belter -Fire Bird Yoga**, its instructor(s), assistants, and hosting facilities from any claims or liability arising from my participation, except where caused by gross negligence or intentional misconduct. Participant Agreement I have read and understood this waiver. I acknowledge that I am participating voluntarily and understand that by signing this document, I am waiving certain legal rights to the extent permitted by law. Please inform your instructor before class if you are pregnant, have a recent injury, surgery, high blood pressure, heart condition, dizziness, osteoporosis, joint replacement, or any other medical condition that may affect safe participation.
